Q. How can I make a gift to the CMAF?

       By check – make checks payable to the Columbus Medical Association Foundation
       and mail to:

       CMAF
       1390 Dublin Rd.
       Columbus, OH 43215
 Provide fund name or purpose of gift in memo section of check.

By credit card – go online to: CMAFOhio.org/donatenow and follow the prompts; or contact the Foundation by phone at (614) 240-7420 and provide credit card information.

With securities – provide instructions to your broker regarding # of shares and name of security to be gifted and have your broker contact the Foundation at (614) 240-7420 for transfer instructions.

For other ways to give, including bequests, life insurance, real estate, retirement plan assets and charitable trusts, contact the Foundation at (614) 240-7420 for assistance.

Q. What is the YAC?

The Youth Advisory Council is the Foundation’s youth philanthropy group made up of middle and high school students from Central Ohio. The council makes grants to fund youth focused health-related programs and projects at local non-profits, youth groups and and schools.

Q. Why was the YAC created?

The YAC was created in January 2001 to teach youth about healthcare philanthropy and assist the Columbus Medical Association Foundation in its grantmaking that targets youth.

Q. What size grants or sponsorships are available through the YAC?

The YAC provides grant and sponsorship support up to $2,000.
