Foundation Priority: Supporting and Improving Healthcare Safety Net Clinical Services


CMAF’s goal is to improve the provision and coordination of and the access to acute primary and specialty healthcare services for Central Ohio’s uninsured, underinsured and most vulnerable population.

Grantee Partner – Physicians CareConnection (PCC)

PCC is a volunteer-based healthcare organization that specializes in providing and coordinating healthcare and related social services that directly influence the quality of life for people who are vulnerable.

They operate a free clinic that provides medical, dental and vision services and links patients to physicians in private offices. They provide care coordination that connects patients to healthcare and community resources including:

  • dental care
  • prenatal care
  • primary care
  • specialty care
  • vision services
  • health insurance enrollment
  • medical diagnostic testing and labs
  • medical interpretation
  • prescription assistance
  • family coaching and support
  • linkages to food services and nutrition
